Oxygen Saturation Scales
Oxygen saturation scales are used to assess the amount of oxygen carried by haemoglobin in the blood, providing healthcare professionals with a rapid indication of a patient’s respiratory status and oxygenation. Measured as peripheral oxygen saturation (SpO₂), this value is obtained non-invasively using a pulse oximeter and is a routine component of patient assessment in primary care, emergency medicine, hospital wards, and critical care.
SpO₂ is expressed as a percentage, representing the proportion of haemoglobin molecules carrying oxygen. In healthy adults breathing room air, oxygen saturation typically ranges between 95% and 100%, although target values may differ depending on the patient’s underlying medical conditions.
Oxygen saturation scales are commonly incorporated into clinical assessment tools and early warning systems to help identify patients at risk of respiratory deterioration. A falling oxygen saturation may indicate impaired gas exchange, respiratory disease, circulatory compromise, or other serious medical conditions requiring further investigation.
Many healthcare organisations use defined oxygen saturation target ranges to guide oxygen therapy. For most acutely ill adults, a target SpO₂ of 94–98% is commonly recommended. Patients with chronic hypercapnic respiratory conditions, such as chronic obstructive pulmonary disease (COPD), may require lower target ranges, typically 88–92%, to reduce the risk of carbon dioxide retention. Clinical decisions should always follow local guidelines and individual patient requirements.
Pulse oximetry provides valuable real-time information but also has limitations. Factors such as poor peripheral perfusion, patient movement, nail polish, skin pigmentation, severe anaemia, carbon monoxide poisoning, and equipment performance can influence measurement accuracy. Oxygen saturation should therefore always be interpreted alongside the patient’s clinical presentation, respiratory rate, heart rate, and other vital signs.
Oxygen saturation scales form an important part of many clinical scoring systems, including early warning scores, sepsis assessments, and respiratory risk stratification tools. They help clinicians identify patients who may require supplemental oxygen, further investigation, or escalation of care.
While oxygen saturation is an essential physiological measurement, it represents only one aspect of respiratory assessment. Comprehensive clinical evaluation, including history, examination, and appropriate investigations, remains fundamental to the diagnosis and management of patients with suspected respiratory compromise.
By providing a rapid, objective measure of oxygenation, oxygen saturation scales support timely clinical decision-making and contribute to the early recognition of deterioration across a wide range of healthcare settings.
