Serum Calcium
Serum calcium is an essential laboratory measurement used to assess calcium levels in the blood. Calcium plays a fundamental role in many physiological processes, including muscle contraction, nerve transmission, blood clotting, hormone secretion, and the maintenance of healthy bones and teeth. Because of its importance, serum calcium testing is routinely performed in both primary and secondary care across a wide range of clinical conditions.
Most calcium circulating in the bloodstream is bound to proteins, primarily albumin, while a smaller proportion exists as free (ionised) calcium, which is the biologically active form. Standard laboratory tests usually measure total serum calcium, meaning that abnormal albumin levels can influence the reported result. For this reason, healthcare professionals may calculate corrected calcium to estimate the calcium concentration after accounting for variations in serum albumin.
Abnormal serum calcium levels may indicate a wide variety of underlying medical conditions. Elevated calcium levels, known as hypercalcaemia, may occur in disorders such as primary hyperparathyroidism, malignancy, vitamin D excess, prolonged immobilisation, or certain medications. Patients may present with symptoms including fatigue, constipation, excessive thirst, confusion, muscle weakness, or cardiac rhythm disturbances.
Low serum calcium, or hypocalcaemia, may result from vitamin D deficiency, chronic kidney disease, hypoparathyroidism, pancreatitis, severe illness, or magnesium deficiency. Symptoms can include muscle cramps, paraesthesia, tetany, seizures, and, in severe cases, life-threatening cardiac complications.
Interpretation of serum calcium should always consider the patient’s clinical condition, serum albumin concentration, renal function, vitamin D status, phosphate levels, and parathyroid hormone measurements where appropriate. In critically ill patients or where precise assessment is required, direct measurement of ionised calcium may provide a more accurate reflection of physiologically active calcium than total serum calcium alone.
Serum calcium is incorporated into several clinical calculators and assessment tools, most notably corrected calcium calculators, which help clinicians interpret calcium concentrations in patients with abnormal albumin levels. These tools support clinical decision-making but should always be used alongside laboratory findings, patient history, and professional clinical judgement.
As a routine but clinically significant investigation, serum calcium remains an important biomarker in the diagnosis and management of endocrine disorders, renal disease, metabolic abnormalities, and many acute and chronic medical conditions. Accurate interpretation contributes to timely diagnosis, appropriate treatment, and improved patient care.
