Wells Score for Deep Vein Thrombosis (DVT)
The Wells Score for Deep Vein Thrombosis (DVT) is a validated clinical prediction rule used to estimate the probability that a patient has a lower limb deep vein thrombosis. Developed by Dr. Philip Wells and colleagues, the score combines clinical findings and risk factors to help healthcare professionals determine the likelihood of DVT and guide subsequent investigation.
Deep vein thrombosis occurs when a blood clot forms within the deep veins, most commonly in the legs. Early recognition is important because untreated DVT can lead to serious complications, including pulmonary embolism (PE), post-thrombotic syndrome, and recurrent venous thromboembolism.
The Wells Score assigns points to a range of clinical features associated with DVT, such as active cancer, recent immobilisation or surgery, localised tenderness along the deep venous system, swelling of the entire leg, calf swelling, pitting oedema, collateral superficial veins, previous DVT, and whether an alternative diagnosis is at least as likely as DVT. The total score is then used to classify the patient’s clinical probability as low, moderate, or high risk, or according to simplified two-level models used in many clinical guidelines.
In modern clinical practice, the Wells Score is frequently used alongside D-dimer testing and compression ultrasonography. Patients with a low clinical probability and a negative D-dimer test may not require diagnostic imaging, whereas those with a higher probability or positive D-dimer result generally require further investigation. This structured approach helps reduce unnecessary imaging while ensuring that patients at significant risk receive timely assessment.
The Wells Score is intended to support clinical decision-making rather than provide a definitive diagnosis. The result should always be interpreted alongside the patient’s history, physical examination, laboratory findings, and imaging where appropriate. Clinical judgement remains essential, particularly in patients with atypical presentations or significant comorbidities.
