Adjusted body weight is a dosing-weight estimate sometimes used for selected medicines in people whose actual weight substantially exceeds ideal body weight.

Overview

Adjusted body weight attempts to account for partial distribution of some medicines into excess body mass. This implementation uses Devine IBW and a 0.4 correction factor.

Clinical Significance

The appropriate dosing weight depends on the medicine, indication, pharmacokinetics and local protocol. Adjusted body weight is not universally appropriate.

When to Use

  • Use only when a medicine-specific or local protocol recommends adjusted body weight.
  • Confirm the correction factor required by the protocol.
  • Use current measured height and weight.

How It Is Calculated

  • Adjusted body weight = IBW + 0.4 × (actual weight − IBW).
  • IBW is calculated with the Devine formula before applying the 0.4 adjustment.

Interpretation

  • The result is a dosing-weight estimate.
  • It is not a target or healthy body weight.
  • Use actual, ideal or adjusted weight according to the specific medicine protocol.

Worked Example

If actual weight is 100 kg and IBW is 70 kg, adjusted body weight is 82 kg.
