Lean body weight is an estimated body-size scalar used in selected pharmacokinetic and clinical applications.

Overview

This calculator uses the Janmahasatian equations, which were developed to behave more consistently than some older formulas at extremes of body size.

Clinical Significance

Some medicines and physiological models use lean body weight, but the correct weight scalar is medicine- and protocol-specific.

When to Use

  • Use when the relevant protocol specifies the Janmahasatian equation.
  • Use as an estimate rather than a measured body-composition result.
  • Confirm whether the protocol instead requires ideal, adjusted or actual body weight.

How It Is Calculated

  • Male LBW = (9270 × weight) ÷ (6680 + 216 × BMI); female LBW = (9270 × weight) ÷ (8780 + 244 × BMI).
  • BMI is calculated from current height and weight before applying the sex-specific equation.

Interpretation

  • The result is an estimated lean body weight in kilograms.
  • It should not exceed actual body weight.
  • Its clinical meaning depends on the application.

Worked Example

A 170 cm, 100 kg female has BMI 34.6 and estimated lean body weight of approximately 53.8 kg.

Patient Considerations

  • The equation may be less reliable in pregnancy, oedema, amputation, extreme muscularity and unusual body composition.

Limitations

  • The equation is an estimate, not a direct measurement.
  • It uses binary sex-specific models.
  • Clinical validation is application-specific.
